The Recovery Support Navigator is responsible for providing clinical case management to engage, educate, and support patients seeking treatment upon release and their families. The goal is to motivate and maintain them in culturally relevant behavioral health services in the community. This engagement begins 120 days pre-release and continues up to 90 days post-release. The role involves working independently and as part of a team, assisting patients with transportation and housing resources, and making referrals as needed. The position is primarily on-site unless working with patients in the community. Responsibilities include coordinating necessary treatment for various patient needs and communicating with all referral sources and treatment entities using proper releases of information. Working hours are required to meet the business needs of the organization, and travel to other programs or locations may be necessary.
