Dittmer, MO Full Time Doing Well, By Doing Good At Sana Lake Recovery, we treat substance use and mental health disorders as a chronic disease that is unique to each of our members. Requirements
- LMSW, PLPC, LPC, or LCSW
- Previous work experience with dual diagnosis
- Knowledge about substance use and self-help meetings
- Familiarity with electronic health records
Responsibilities
- Perform various member care activities as directed by the Clinical Director.
- Be responsible for a caseload and provide various clinical services, such as individual trauma and co-occurring therapy, and group trauma counseling.
- Conduct psychosocial and bio-psychosocial assessments and develop a diagnosis from the information gathered in the assessment.
- Collaborate with the Treatment Team to formulate the Master Treatment Plan.
- Assist in coordinating discharge planning.
- Maintain communication between treatment team members, family, referral sources, and other service providers as needed.
