About The Position

We are looking for a compassionate and motivated Therapist (LCSW, LPC) to join our team. You’ll work side-by-side with our providers to provide behavioral health support to our patients in a collaborative and caring environment.
Requirements
- Master’s degree in social work, counseling, or related field.
- Current state license (LCSW, LPC)
- Empathetic, proactive, and team-oriented approach.
Nice To Haves
- Experience in primary care a plus.
Responsibilities
- Partner with primary care providers to identify and support patients’ behavioral health needs.
- Provide therapy, crisis intervention, and care coordination.
- Develop treatment plans that promote overall wellness and health behavior change.
- Document and communicate effectively
