About The Position

The Centers is seeking a compassionate and skilled clinician for the role of Professional III, Behavioral Health Therapist, Permanent Supportive Housing (PSH). This full-time, exempt position is based in Cleveland, Ohio, within the Behavioral Health Program. The role focuses on providing integrated clinical services to adults living with serious mental illness, particularly those with schizophrenia-spectrum disorders, who reside in permanent supportive housing. The therapist will combine direct therapeutic care, case coordination, crisis support, and community-based outreach within a trauma-informed, recovery-oriented framework. The position emphasizes client autonomy and strengths, working as part of a multidisciplinary team that includes housing staff, case managers, medical providers, and community partners to ensure holistic, person-centered care across clinical and housing environments.

Requirements

An active LSW or LPC license in the State of Ohio is required.
A Master's degree in Social Work or a related social service discipline is required.
A minimum of three years of experience in mental health and chemical dependency is required.
Proficiency in clinical acumen, including a deep understanding of schizophrenia-spectrum and other psychotic disorders, and evidence-based treatment approaches and recovery models.
Ability to engage clients with empathy and respect, supporting strengths-based recovery, autonomy, and dignity (Trauma-Informed and Recovery-Oriented Practice).
Skill in building rapport, conducting assessments, and delivering psychotherapy through effective verbal and nonverbal communication (Therapeutic Communication).
Demonstrated competence in identifying and managing psychiatric crises involving suicide risk, acute psychosis, and hospitalization needs (Crisis Management and Risk Assessment).
Proven ability to work effectively across housing, case management, medical, and external agency partners (Interdisciplinary Collaboration).
Awareness, sensitivity, and skill in working respectfully with individuals from diverse cultural, personal, and community backgrounds (Cultural Humility).

Responsibilities

Provide individual and group therapy using evidence-based practices tailored to adults with serious mental illnesses, including schizophrenia-spectrum disorders.
Conduct comprehensive mental health assessments and develop individualized treatment plans in collaboration with clients and the care team, centering client voice and preference.
Deliver services with a recovery-oriented, trauma-informed, and person-centered approach that supports autonomy, dignity, and community integration.
Coordinate care across behavioral health, medical, and housing systems, acting as a liaison among providers, case managers, housing staff, and families.
Address behavioral health needs that impact tenancy, including crisis intervention, symptom management, and medication adherence.
Collaborate actively with housing staff and property management to support clients in maintaining housing stability and accessing wraparound services.
Provide crisis intervention and risk assessment, including suicide and violence risk evaluation.
Coordinate access to higher levels of care when clinically indicated, responding with urgency and compassion in emergency situations.
Maintain accurate and timely documentation in compliance with agency, funding, and licensing standards.
Participate in regular interdisciplinary team meetings to ensure continuity of care and shared decision-making.
Engage in ongoing training and professional development to maintain licensure and stay current on best practices in behavioral health and supportive housing.
