About The Position

Responsible for ensuring that appropriate services are rendered to patients on the unit and in outpatient services. Must work well independently and in combination with a multidisciplinary team to provide efficient and effective treatment. Assesses needs and works directly with the patient on the completion of treatment plan goals. Provides counseling, education, and support to patients and their families in individual and group sessions. Must have excellent written and verbal communication skills for documentation and therapeutic interaction purposes. Periodically provides in-service training to staff on the unit. Interacts with Physicians and community agencies to provide continuity of care for the prevention of recurring problems and case management discharge planning. Serves as a representative of the Behavioral Health Center by presenting community education programs. This requires excellent organizational and presentation skills as well as extensive knowledge of mental health.

Requirements

  • Master’s degree in social sciences, social work, psychology or a related field.
  • One or more years of experience in behavioral health.
  • Crisis Response Training, within 60 days of hire.

  • Implementing the plan in a safe and timely manner.
  • Documenting implementation and including changes or omissions of the identified plan.
  • Consulting with healthcare providers and making referrals as appropriate.
  • Utilizing appropriate community resources and systems to implement the plan.
  • Treating patients and families with compassion and implementing plan of care.
  • Basing care decisions on plan of care.
  • Monitoring and documenting patient response.
  • Establishing rapport with clients.
  • Adapting communication style, listening and responding to patient needs.
  • Recognizing age-specific responses to treatment.
  • Providing effective interventions.
  • Respecting patients confidentiality and privacy.
  • Supporting social, spiritual, and cultural needs of patients and families.
  • Managing difficult or emotional patient situations.
  • Documenting and submitting required information and data in a timely fashion.
  • Clearly and accurately documenting designated processes, policies, products, service offerings, etc.
  • Ensuring that documentation is tailored to expected readers / users.
  • Using correct terminology.
  • Conforming to required style and format.
  • Developing expected outcomes that provide direction for continuity of care.
  • Involving patient/family and other disciplines to determine expected outcomes.
  • Modifying expected outcomes based on changes in the assessment of the patient.
  • Including the patient and others involved in the care in the evaluation process.
  • Evaluating the plan in relation to patient responses and expected outcomes.
  • Documenting the results of the evaluation.
