About The Position

Belle is redefining primary care for Medicare members by integrating in-home preventive foot care with AI-powered telehealth and care coordination. Our Community Health Workers, who are also licensed nail technicians, build trusted relationships during in-home visits, identifying health issues early and connecting members to Chronic Care Management. This model aims to provide a more engaging primary care experience, helping seniors stay healthier and age in place. Belle has achieved a high NPS score, indicating strong patient trust and loyalty. We are seeking a Board Certified (or Eligible) Primary Care Physician to deliver high-quality, preventive-focused care through telehealth, improving health outcomes and making care more accessible and joyful for America's seniors.

Requirements

MD or DO, Board Certified/Eligible in Family Medicine, Internal Medicine, or Geriatrics
Active medical license — multi-state licensure a plus
Credentialed with major plans: UHC, Aetna, Humana
Strong interest in preventive care, senior health, or chronic condition management
Tech-savvy and comfortable practicing via telehealth
Collaborative, mission-driven mindset with excellent communication skills

Responsibilities

Provide routine telehealth visits with Medicare patients (E/M and Chronic Care Management)
Create and manage personalized care plans for ongoing conditions
Collaborate with our integrated care team (nurse case managers, NPs, CHWs)
Ensure thorough documentation in our EHR and compliance with quality measures
Engage patients in preventive care to reduce avoidable complications
