About The Position

Solace connects patients with healthcare advocates who help them navigate access to care. The physician builds strong patient relationships and facilitates an exceptional advocacy experience, collaborating with healthcare advocates to help patients achieve their health goals and support better outcomes. The role contributes to developing a new patient care program designed to serve a vulnerable population. This position is for physicians residing in Minnesota and requires a DEA license and HAP.

Requirements

• Reside in Minnesota.
• Hold a DEA license and HAP.
• Be board certified in family medicine, internal medicine, or another relevant specialty.
• Have strong clinical and organizational skills, with the ability to balance multiple tasks and work under pressure while maintaining organized communications and documentation.
• Have excellent communication skills, including listening and empathy.
• Be comfortable working with elderly patients and individuals experiencing social determinants of health.
• Be technically savvy and able to quickly learn new systems and software.
• Demonstrate a strong bias toward action and execution and be willing to provide feedback.
• Be comfortable with rapid changes to the business, strategy, organization, and priorities.
• Be based in the United States.

Responsibilities

• Learn Solace's systems, tools, technology, partners, and expectations while providing clinical expertise in every interaction.
• Identify and prioritize patients' needs and maintain a streamlined care continuum.
• Oversee patient care plans in partnership with healthcare advocates, addressing social determinants of health such as food resources, transportation access, and support at home.
