About The Position

Requirements

• Reside in Mississippi.
• Hold a DEA license and HAP.
• Be board certified in family medicine, internal medicine, or another relevant specialty.
• Have strong clinical and organizational skills, with the ability to balance multiple tasks and work under pressure while maintaining organized communications and documentation.
• Have superior communication skills, including listening and empathy.
• Be comfortable working with elderly patients and individuals experiencing social determinants of health.
• Be technically savvy and able to learn new systems and software quickly.
• Show a strong bias toward action and execution and be willing to share feedback.
• Embrace change and iteration, including rapid changes to the business, strategy, organization, and priorities.
• Be based in the United States.

Responsibilities

• Learn Solace’s systems, tools, technology, partners, and expectations, while providing clinical expertise in each interaction.
• Identify and prioritize patients’ needs and maintain a streamlined care continuum.
• Oversee patient care plans in partnership with healthcare advocates, addressing social determinants of health such as food resources, transportation access, and support at home.
• Build strong patient relationships and facilitate an effective advocacy experience.
• Contribute to the development of a patient care program.
