Sprenger Therapy is seeking an Ohio-licensed Physical Therapist to provide telehealth services to residents in its skilled nursing communities. The position delivers patient-centered care through secure video technology while collaborating with on-site therapy teams. Therapists work remotely as part of an in-house rehabilitation organization, with clinical and operational support, professional autonomy, and a team-oriented environment. The role is based within Sprenger’s Ohio communities, although the job details list SHC of Bluffton in Okatie, South Carolina. The Physical Therapist conducts evaluations and reassessments, develops individualized plans of care, and provides skilled treatment focused on mobility, strength, balance, safety, function, and independence. The therapist collaborates with residents, families, on-site therapy staff, nursing staff, physicians, and other care team members; may supervise Physical Therapist Assistants as permitted by Ohio law and payer requirements; and participates in care and discharge planning. Clear documentation and a personal, engaging, resident-centered telehealth experience are expected. The role requires a degree in Physical Therapy from an accredited program and a current Ohio Physical Therapist license or eligibility for Ohio licensure. Experience in skilled nursing, geriatrics, post-acute care, or telehealth is preferred. Experienced clinicians are preferred, though new graduates with strong geriatric clinical experience and the ability to practice independently may be considered. Candidates need reliable high-speed internet and a private workspace suitable for confidential healthcare communication.
