About The Position

This is a remote role for a Telehealth Nurse Practitioner in California. The position involves delivering telehealth visits on a flexible schedule, with a minimum of 24 hours per week. The role offers the opportunity to own your panel, work with clear workflows, and eliminate commuting. You will conduct comprehensive health assessments, document HCC (risk adjustment) during visits, close HEDIS (quality measures) care gaps, review patient history, medications, and preventive needs, code with ICD-10 and CPT II, deliver care plans and follow-up, and maintain clean, audit-ready documentation.

Requirements

• Active California Nurse Practitioner license
• 1+ year Family or Internal Medicine experience
• Medicare and Medicaid experience
• Familiarity with HEDIS and risk adjustment
• Provider enrollment for Medicare/Medicaid
• Comfortable with telehealth platforms
