About The Position

Our client is a fast-growing healthcare organization supporting Medicare and Medicaid populations through virtual care. This role involves delivering telehealth visits from home, conducting comprehensive health assessments, documenting risk adjustment (HCC coding), closing HEDIS care gaps, reviewing medical history, medications, and preventive needs, documenting visits using ICD-10 and CPT II codes, and delivering clear care plans and follow-up guidance. The position requires maintaining accurate visit documentation for quality programs. Your work expands preventive care access for Medicare and Medicaid members, helps close care gaps and improve quality scores across populations. You are expected to like pace and ownership, and to start, finish, and follow through.

Requirements

• Active Georgia Nurse Practitioner license
• 1+ year Family Medicine or Internal Medicine experience
• Experience with Medicare and Medicaid populations
• Familiar with HEDIS and risk adjustment workflows
• Medicare and Medicaid provider enrollment required
• Comfortable delivering care through telehealth platforms
