About The Position

This role involves delivering telehealth visits from home for individuals in California. The Nurse Practitioner will be responsible for conducting comprehensive health assessments, documenting HCC (risk adjustment), closing HEDIS (quality measures) care gaps, reviewing patient history and medications, coding with ICD-10 and CPT II, delivering care plans, and maintaining audit-ready documentation. The position offers a flexible schedule with a minimum of 24 hours per week and a steady flow of visits.

Requirements

• Active California Nurse Practitioner license
• 1+ year Family or Internal Medicine experience
• Medicare and Medicaid experience
• HEDIS and risk adjustment familiarity
• Medicare/Medicaid provider enrollment
• Comfortable with telehealth platforms
