As a Telehealth Coordinator, you will serve as the primary point of contact for patients throughout their home health journey. You will provide clinical support, proactive patient outreach, care coordination, and healthcare navigation while partnering with branch clinicians, physicians, caregivers, and interdisciplinary teams to improve patient outcomes. This role combines inbound clinical support with proactive patient engagement to reduce avoidable hospitalizations, improve patient satisfaction, identify barriers to care, and ensure timely communication across the care continuum. Your work allows field clinicians to spend more time providing direct patient care while ensuring every patient receives compassionate, coordinated support.
