About The Position

Coastal Horizons Center is seeking a dedicated and qualified TASC Care Manager to join their team. This position offers a unique opportunity to support justice-involved individuals by providing case management and service coordination within the TASC (Treatment Accountability for Safer Communities) program. The ideal candidate will collaborate with community partners, justice systems, and service providers to help individuals access treatment and recovery services. This specific opportunity is located in Durham County (Durham, NC).

Requirements

Bachelor's Degree or higher in a human services or related field
Knowledge of recovery, harm reduction, and wellness-based concepts
Strong communication, organizational, and time-management skills
Ability to work independently and manage a caseload effectively
Proficiency in the Microsoft Office Suite and similar platforms
Proficiency in using electronic health record systems and remote collaboration tools (i.e., Teams, Zoom, etc.)
Demonstrated ability to collaborate with diverse stakeholders, including legal systems, healthcare providers, and social services

Nice To Haves

North Carolina Peer Support Specialist certification
Minimum of 2 years of experience
Individuals registered with the North Carolina Addiction Specialist Professional Practice Board (NCASPPB)
May have lived experience with recovery and/or the justice system

Responsibilities

Administering psycho-social screenings/assessments
Coordinating linkage to behavioral health treatment as well as medical, educational, and vocational services as needed
Providing ongoing professional care management services for the justice-involved population
Collaborating with community partners, justice systems, and service providers to help individuals access treatment and recovery services
