About The Position

Horizon is seeking a Targeted Case Manager to join their team in the Mental Health and Addictions field. The role involves working in a supportive, team-based environment with a seamless onboarding experience. The Targeted Case Manager will be responsible for assessing patients' social determinants of health, participating in the Open Access process for rapid care access, and providing patient support through various outreach methods. This includes connecting patients to community resources, delivering transitional care from inpatient or residential settings, and assisting individuals in addressing gaps in care and achieving health goals. The role also requires utilizing clinical and non-clinical data from EHR systems and building positive relationships with individuals with behavioral health disorders and their families, while adhering to Horizon Health Services policies and privacy regulations.

Requirements

  • Bachelors degree and 6 months of relevant experience required. Relevant experience includes previous work in a behavioral health setting in a patient facing role
  • Associates degree and 1 year of relevant experience required. Relevant experience includes previous work in a behavioral health setting in a patient facing role
  • High School Diploma/equivalent and 2 years of relevant experience required. Relevant experience includes previous work in a behavioral health setting in a patient facing role
  • Computer navigation skills, proficiency in Microsoft office skills.
  • Experience working within Electronic Health Record (EHR) systems and willingness to learn new software applications preferred.
  • Familiarity with reporting and entry level data analytics.

Responsibilities

  • Complete Social Determinants of Health (SDOH) assessments for patients.
  • Participate in the Open Access process to assist patients with rapid access to care.
  • Provide patient support through outreach calls, telehealth, and face-to-face visits to identify barriers and connect patients to community resources and services.
  • Deliver transitional care, including follow-up outreach to connect patients from inpatient, emergency room, or residential treatment settings to appropriate providers.
  • Connect individuals with necessary medical, social, legal, housing, and other supportive services.
  • Assist patients in addressing gaps in care and achieving health goals.
  • Retrieve and utilize clinical and non-clinical data from internal Electronic Health Record (EHR) systems and regional health information networks.
  • Build positive, supportive relationships with individuals who have behavioral health disorders and their families, respecting diverse ethnic, cultural, and socioeconomic backgrounds.
  • Learn and adhere to Horizon Health Services policies and procedures, including compliance with privacy and confidentiality regulations.
