As a Targeted Case Manager at Horizon, you will complete Social Determinants of Health (SDOH) assessments for patients, participate in the Open Access process to assist patients with rapid access to care, and provide patient support through outreach calls, telehealth, and face-to-face visits to identify barriers and connect patients to community resources and services. You will also deliver transitional care, including follow-up outreach to connect patients from inpatient, emergency room, or residential treatment settings to appropriate providers. Additionally, you will connect individuals with necessary medical, social, legal, housing, and other supportive services, assist patients in addressing gaps in care and achieving health goals, retrieve and utilize clinical and non-clinical data from internal Electronic Health Record (EHR) systems and regional health information networks, and build positive, supportive relationships with individuals who have behavioral health disorders and their families, respecting diverse ethnic, cultural, and socioeconomic backgrounds. You will learn and adhere to Horizon Health Services policies and procedures, including compliance with privacy and confidentiality regulations.

Requirements

  • Bachelors degree and 6 months of relevant experience required. Relevant experience includes previous work in a behavioral health setting in a patient facing role - OR Associates degree and 1 year of relevant experience required. Relevant experience includes previous work in a behavioral health setting in a patient facing role - OR High School Diploma/equivalent and 2 years of relevant experience required.   • Computer navigation skills, proficiency in Microsoft office skills.
  • Experience working within Electronic Health Record (EHR) systems and willingness to learn new software applications preferred.
  • Familiarity with reporting and entry level data analytics.

Responsibilities
