Tailored Care Management Supervisor
About The Position
Easterseals PORT Health is seeking a Supervisor to guide their Tailored Care Management team with compassion, clarity, and commitment. This position is based in Pitt County and focuses on delivering whole-person care to individuals living with mental health challenges, substance use disorders, and disabilities. The role involves providing strategic oversight and inspiring leadership to a team that makes a measurable difference in the lives of clients. The organization has been recognized as Raleigh’s Best Nonprofit Organization and Raleigh’s Best Mental Health Services for two consecutive years. The Supervisor will champion innovative care, mentor Care Managers, drive program excellence through strategic planning, quality improvement, compliance, and fiscal oversight, and foster collaboration and creativity while prioritizing both clients and staff.
Requirements
- A Master’s degree in a human services field
- Full or provisional license: LCAS, LMFT, LCSW, LCMHC, or RN (REQUIRED)
- At least 3 years of experience in care management, coordination, or case management
- A valid driver’s license and insurance for work travel
Nice To Haves
- Minimum of one (1) year of supervisory or team lead experience preferred
- Strong communication, crisis de-escalation, and strategic problem-solving skills
