About The Position

The SW Student Discharge Planner works under the supervision of the Manager of Social Services. The role supports patients and their families, focusing on discharge planning and making appropriate referrals based on patients’ ages and needs. The employee also performs other duties as assigned.

Requirements

High school diploma or successful completion of an equivalent high school exam.
Bachelor's degree.
Enrolled in a master's degree program or completed a master's degree in Social Work.
BLS certification within 90 days of hire.

Responsibilities

Arrange continuing care after discharge, including coordinating services with community resources.
Complete assessments and follow-ups for patients and families.
Serve as a liaison among patients, families, physicians, advanced care practitioners, organizations, and outside agencies.
Respond to high-risk screenings and physician consults according to established criteria.
Maintain accurate records in patient charts and departmental statistics.
Participate in team meetings, non-departmental meetings, and ongoing hospital committees.
Participate in departmental Quality Improvement programs.
Maintain skills through continuing education and current knowledge of community resources.
Arrange discharge planning needs by meeting and speaking with patients and families and collaborating with providers, nursing, therapy, and other skilled disciplines.
