Under the direction of the Supportive Housing Manager, the Case Manager acts as clinical lead and one of the senior practitioners of the Supportive Housing 24-hour care setting that is consistent with the vision, mission, and core values established by Dilico Anishinabek Family care. The Case Manager will be primarily responsible for engagement, stabilization with a strong focus on transition and aftercare planning. Responsibilities will be to provide clinical consultation and guidance to the Supportive Housing program team and to ensure continued integration with other internal programs and community partners. Incorporating cultural aspects into the stabilization plan and supportive housing environment as well as case conferencing with external partners for a successful transition out of the program and with First Nations will also be a part of this role.
