About The Position

The Supported Living Program at Community Vision enables individuals with developmental disabilities to live independently in their chosen homes. This is achieved by assisting with general life activities such as finances, transportation, medical needs, and social engagement, providing one-on-one direct support, and offering emergency assistance. The Supported Living Manager is crucial in ensuring that individuals in the program have their basic needs met, access resources that enhance their quality of life, and receive support to make informed decisions. This role involves coaching direct support staff and coordinating services that promote choice, community integration, and personal relationships.

Requirements

  • Five (5) years of experience in the developmental disabilities field with one year in a supervisory role -- or a bachelor’s degree with at least one-year paid experience in developmental disabilities field, or human or community development services, required.
  • One (1) year supervisory, case management, or program management experience, required.
  • Must have clean driving record and maintain safe vehicle with minimum required auto insurance.
  • Must complete and maintain CPR/First Aid, Oregon Intervention System (OIS), and other job-related training as mandated by the organization or state regulations.
  • Must successfully pass a criminal history check through the Oregon DHS Background Check Unit and every two years thereafter as a condition of continued employment.
  • Must have and maintain a clean driving record, which will be verified through the Department of Motor Vehicles (DMV) upon hire and periodically thereafter.
  • Must not appear on the Office of Inspector General (OIG) List of Excluded Individuals and Entities as a condition of continued employment.
  • Ability to perform all aspects of the Support Companion (i.e., DSP) role.

Responsibilities

  • Advocates for the expressed goals of people supported as identified by them.
  • Ensures the safety of people receiving services and their staff.
  • Responds to crisis in emergency situations.
  • Provides direct support as needed.
  • Assures medical needs are met in a timely and comprehensive manner.
  • Helps supported person manage personal finances.
  • Assumes responsibility for supervision, coaching, and appraising performance of direct support staff.
  • Communicates with families, case management personnel, other providers, and the community at large as an advocate for the people supported.
  • Coordinates staff activities with the person supported.
  • Facilitates team meetings and team-building.
  • Ensures that staff are trained to perform responsibilities.
  • Participates in Community Vision's on-call support system to ensure staffing needs are met 24-hours/day.
  • Maintains required documentation and reports.
  • Assists with compliance requirements with all program, agency, and government policies and procedures.
