About The Position

This is a full-time, para-professional level position responsible for teaching, training, and assisting individuals in developing skills for independent living. The role involves coordinating health and medical needs, fostering community participation, assisting with advocacy, preparing diet plans, writing intervention plans, providing safety skills training, managing personal business and property skills, budgeting, transportation, and daily living skills training. The Specialist will also orient new individuals, assess skills, collect data, supervise finances, provide a supportive atmosphere, train new staff, and ensure residences meet cleanliness, safety, and health standards. This role requires adherence to schedules, maintaining confidentiality, and completing all necessary documentation.

Requirements

Must not have or be a carrier of serious infectious or communicable disease.
18 years of age.
High school diploma or GED.
Exercise sound judgment when the situation requires immediate decision-making.
Obtain and maintain a valid Iowa Chauffeur’s license.
Maintain acceptable driving record as determined by agency insurance carrier.
Successful completion of pre-employment physical, TB Tine, and drug screenings.
Complete 10-hour med-management class.
Must be willing to work with individuals who have a behavioral disorder, which may result in the display of aggressive outbursts of a physical nature.
Must be willing to clean any biological fluids/waste, such as, but not limited to blood, feces, urine, or vomit.
Must have the ability and desire to work with other employees as a team.
Must have good verbal and written communication skills.
Must be self-motivated.
Must not have been legally convicted of any type of assault, abuse, or bodily injury.
All agency staff are required to participate and cooperate with agency investigations.
Must be able to travel from place to place, up to 70 miles.
Must be able to enter and exit resident homes that may or may not be accessible to everyone.
Must be able to walk up and down stairs with hands full to perform laundry duties, etc.
Must be able to work a full scheduled shift as assigned.
Must be able to carry necessary supplies up to 50 pounds to perform job duties.
Must be able to provide physical care necessary to residents when performing direct care duties.
Ability to physically assist an individual to regain his balance or to transfer from a sitting, standing or prone position.
Demonstrate ability to perform routine household tasks, such as, but not limited to vacuuming, mopping, and the use of cleaning agents, meal preparation, kitchen clean up, and clothing care.
Must be able to assist an individual in a wheelchair to transfer to another chair, bed, toilet, or bath.
Must be able to assist someone in the shower or bath.
Must be able to make multiple trips from car to house carrying purchases such as groceries, etc.
Must be able to perform snow removal as necessary for individual and staff safety.

Nice To Haves

Prefer previous similar job experience in teaching skills of cooking, cleaning, and personal hygiene.

Responsibilities

Teach, train, and assist individuals in developing skills for independent living, including activities of daily living.
Coordinate health and medical needs and train individuals to manage their own health.
Develop or maintain skills for better community participation and assist individuals in accessing community services.
Assist individuals in advocacy to ensure their rights and meet their needs, interests, and choices.
Assist in preparing individualized diet plans to meet nutritional needs.
Write Individualized Intervention Plans for each individual served.
Provide emergency/safety skills training.
Provide training in skills necessary to manage personal business and property, including budgeting.
Provide transportation for appointments, church, etc.
Provide training in daily living skills.
Orient new individuals, assess their skills, and provide input for progress reports and Person-Centered Plans (PCP).
Supervise individual goals and complete data collection.
Supervise individual finances to provide guidance in budgeting.
Provide a supportive and therapeutic atmosphere.
Supervise the individual’s care and use of personal property and assist with purchases.
Train new staff on duties, schedules, agency policies, and regulatory policies and procedures.
Follow the assigned work schedule.
Never leave individuals receiving twenty-four-hour care unsupervised.
Complete all records and documentation using proper forms.
Attend staff meetings and in-service training sessions.
Ensure that the individual’s residence meets cleanliness, safety, and health standards.
Stay informed of all programming and administrative information by checking email, logbooks, or other messages.
Keep all written and verbal information on each individual strictly confidential.
Complete documentation of behaviors or provide information for writing staff intervention plans.
Handle other duties as assigned.
Monitor and document an individual’s physician ordered diet.
Assist individuals with activities of daily living such as meal preparation, laundry, housekeeping, toileting, and bathing.
Ensure clients are safe from all environmental hazards.
Provide quality care.
Administer medication and maintain accurate medication documentation, after training.
Exercise sound judgment for immediate decision-making.
Comply with state and federal regulations regarding HIPAA.
Ensure the welfare and safety of persons served in a residential setting.
Work with individuals who have behavioral problems, including verbal and physical aggression.
Clean biological fluids/waste.
