About The Position

This is a full-time position in Malvern, IA, with hours from Monday to Friday, 7:45 am to 3:00 pm. The Supported Community Living Specialist (SCLS) is directly responsible to the assigned Team Leader and the Director of Residential Services. This is a para-professional level position focused on teaching, training, and assisting individuals in developing skills for independence in daily living, community participation, and self-advocacy. The role involves coordinating health and medical needs, assisting with community service access, preparing diet plans, writing intervention plans, providing safety skills training, and managing personal business and budgeting skills. Additionally, the SCLS will provide transportation, orient new individuals, supervise goals and data collection, manage finances, maintain a supportive atmosphere, and train new staff. Adherence to the work schedule, ensuring individuals are never left unsupervised, and completing all documentation are critical. The position requires staying informed of agency information, maintaining confidentiality, documenting behaviors, and handling other assigned duties. The SCLS will also monitor physician-ordered diets and assist individuals with daily living activities such as meal preparation, laundry, housekeeping, toileting, and bathing, while ensuring client safety from environmental hazards.

Requirements

  • Individuals must not have or be a carrier of serious infectious or communicable disease.
  • 18 years of age.
  • High school diploma or GED.
  • Exercise sound judgment when the situation requires immediate decision-making.
  • Obtain and maintain a valid Iowa Chauffeur’s license and maintain an acceptable driving record.
  • Successful completion of pre-employment physical, TB Tine, and drug screenings.
  • Complete 10-hour med-management class.
  • Must be willing to work with individuals who have a behavioral disorder, which may result in aggressive outbursts.
  • Must be willing to clean biological fluids/waste, such as blood, feces, urine, or vomit.
  • Must have the ability and desire to work with other employees as a team.
  • Must have good verbal and written communication skills.
  • Must be self-motivated.
  • Must not have been legally convicted of any type of assault, abuse, or bodily injury.
  • All agency staff are required to participate and cooperate with agency investigations.
  • Must be able to travel from place to place, up to 70 miles.
  • Must be able to enter and exit resident homes that may or may not be accessible to everyone.
  • Must be able to walk up and down stairs with hands full to perform laundry duties, etc.
  • Must be able to work a full scheduled shift as assigned.
  • Must be able to carry necessary supplies up to 50 pounds to perform job duties.
  • Must be able to provide physical care necessary to residents when performing direct care duties.
  • Ability to physically assist an individual to regain his balance or to transfer from a sitting, standing or prone position.
  • Demonstrate ability to perform routine household tasks, such as vacuuming, mopping, and the use of cleaning agents, meal preparation, kitchen clean up, and clothing care.
  • Must be able to assist an individual in a wheelchair to transfer to another chair, bed, toilet, or bath.
  • Must be able to assist someone in the shower or bath.
  • Must be able to make multiple trips from car to house carrying purchases such as groceries, etc.
  • Must be able to perform snow removal as much as necessary for individual and staff safety.

Nice To Haves

  • Previous similar job experience in teaching skills of cooking, cleaning, and personal hygiene.

Responsibilities

  • Teach, train, and assist individuals in developing skills for independence in activities of daily living.
  • Coordinate health and medical needs and train individuals to manage their own health.
  • Develop or maintain skills for better community participation and assist individuals in accessing community services.
  • Assist individuals with participation in at least one community activity each week if shift is during day/evening/weekend hours.
  • Assist individuals in advocacy procedures to ensure their rights and meet their needs, interests, and choices.
  • Assist in preparing an individualized diet plan to meet nutritional needs.
  • Write Individualized Intervention Plans for each individual served.
  • Provide emergency/safety skills training.
  • Provide training in skills necessary to manage personal business and property, including budgeting.
  • Provide transportation for appointments, church, etc.
  • Provide training in daily living skills.
  • Orient new individuals, assess their skills, and provide input for progress reports and Person-Centered Plans (PCP).
  • Supervise individual's goals and complete data collection.
  • Supervise individual's finances and provide guidance in budgeting.
  • Provide a supportive and therapeutic atmosphere.
  • Supervise the individual’s care and use of personal property and assist with purchases.
  • Train new staff on duties, schedules, agency policies, and regulatory policies.
  • Follow the assigned work schedule.
  • Never leave individuals receiving twenty-four-hour care unsupervised.
  • Complete all records and documentation using proper forms.
  • Attend staff meetings and in-service training sessions.
  • Ensure the individual’s residence meets cleanliness, safety, and health standards.
  • Stay informed of all programming and administrative information by checking email, daily logbook, or other messages.
  • Keep all written and verbal information on each individual strictly confidential.
  • Complete documentation of behaviors or provide information for writing staff intervention plans.
  • Handle any other duties as assigned by the Team Leader, Lead Staff, and Director of Residential Services.
  • Monitor and document an individual’s physician ordered diet.
  • Assist individuals receiving services with their activities of daily living, such as meal preparation, laundry, housekeeping, toileting, and bathing.
  • Ensure the clients are safe from all environmental hazards.
  • Provide quality care.
  • Administer medication and maintain accurate medication documentation, as needed, after being trained.
  • Exercise sound judgment when immediate decision-making is required.
  • Comply with state and federal regulations regarding HIPAA.
  • Ensure the welfare and safety of persons served in a residential setting.
