Supported Community Living Associate FT
About The Position
Goodwill of the Heartland is hiring a Supported Community Living Associate to support individuals facing barriers to independence. The role develops and maintains relationships with program participants and identifies and uses community resources that may benefit them. It identifies each participant’s treatment and service needs, monitors and evaluates progress on treatment plans, and assists or facilitates services as appropriate. The associate also helps manage expenses and client funding, monitors billable hours, writes high-quality case notes, and assists the Supported Community Living Specialist with service plan development and progress reports. The position provides transportation to participants as needed and requires travel between participant locations. The schedule is Monday through Friday, 8:00 a.m. to 5:00 p.m., for 40 hours per week. The starting wage is $16.78 per hour.
Requirements
- High school diploma or GED.
- Six months of experience in training and/or rehabilitation; post-secondary education may be substituted for this experience requirement.
- Reliable vehicle, valid Iowa driver’s license, and proof of auto insurance.
- Successful completion of a motor vehicle record screening, dependent and adult child abuse screening, and criminal background check.
Responsibilities
- Develop and maintain effective relationships with clients.
- Identify and maintain current information about community resources that may benefit program participants.
- Identify the treatment and service needs of each individual served.
- Monitor and evaluate progress on treatment plans, assisting and facilitating as appropriate.
- Monitor and control expenses and promote efficiency in rehabilitation services, including billable hours and client funding.
- Write quality case notes and assist the Supported Community Living Specialist with ISP development and progress reports.
- Provide transportation to participants as needed.
