About The Position

Relationships Australia South Australia (RASA) is a not-for-profit, secular, community organisation, helping people to overcome difficulties and bring about positive change in their lives. We offer services for families, couples and individuals of all ages, backgrounds and abilities. RASA is seeking a dedicated Support Facilitator to assist young people who are transitioning to independence after experiencing the child protection system. This role requires a focus on developing independent living skills, goal setting and supporting self-determination in life pathways. Your work will involve holistic case management and enabling support collaboration, including screening and assessment, enabling referral pathways, advocacy, and facilitating client-led decision-making processes. Requirements

  • Bachelor’s Degree (AQF Level 7) in Social Work, or demonstrated knowledge and competencies appropriate to Social Work.
  • Experience with complex client matters.
  • Experience in delivering trauma informed practice.
  • Experience working with Aboriginal and Torres Strait Islander young people, families or communities effectively, using culturally responsive approaches and processes.

Nice To Haves

  • Experience working with clients who have experienced Out of Home Care.

Responsibilities

  • Facilitate restorative, client-led decision-making processes with individuals and families accessing the service.
  • Provide trauma-informed, collaborative case management and support coordination to achieve client objectives (including assessment, goal setting, and case work).
  • Undertake universal holistic screening and risk assessment utilising the Detection of Overall Risk Screen (DOORS) tool, as well as other required assessments.
  • Provide referral options and support that promotes independent living skills around financial literacy, housing access, tenancy management, social connection, respectful relationships, cultural connection, and other life domains as identified by the client.
  • Provide direct client services in an accessible manner to Post Care Support Service clients through face-to-face, outreach, telehealth, and by phone.
