About The Position

At Mains'l, we are in the business of hopes and dreams. We are seeking a skilled Direct Support Professional/ Caregiver or lead who is passionate about providing high-quality, person-centered support services that promote independence, growth, and community inclusion. As a Support Coordinator, you would be acting as a role model and mentor to employees using Mains'l Vision, Mission, and Values. Working closely with a Program Manager, you will ensure the residential spaces are running smoothly. This includes ensuring a safe living environment for supported individuals; supporting household tasks, acting as contact with medical professionals, scheduling needed medical and dental appointments, ensuring transportation arrangements are made and more. You'll also be supporting and collaborating with people receiving services and crew working towards personalized goals and/or creating new ones. This position is full-time (40 hours per week), schedule to be determined.

Requirements

  • At least 18 years of age
  • Ability to pass a background check through the Department of Justice (DOJ), and FBI if needed.
  • Safe and reliable transportation with a valid driver’s license and auto insurance.
  • TB (Tuberculosis) Skin Test.
  • Physical, if applicable.
  • The ability to act as a role model to DSPs, utilizing opportunities to engage in person-centered thinking and tools to provide excellent support.
  • The knowledge to discuss documentation guidelines with coworkers to ensure consistency.
  • The skills to accurately document the progression of goals as required for supported persons.
  • The ability to review DSP documentation to ensure accuracy and inform the Program Manager when entries need attention.

Responsibilities

  • Act as a role model and mentor to employees using Mains'l Vision, Mission, and Values.
  • Ensure residential spaces are running smoothly, including a safe living environment for supported individuals.
  • Support household tasks.
  • Act as a contact with medical professionals.
  • Schedule needed medical and dental appointments.
  • Ensure transportation arrangements are made.
  • Support and collaborate with people receiving services and crew working towards personalized goals and/or creating new ones.
  • Follow the Individual Program Plans (IPP) and Person-Centered Descriptions (PCD).
  • Coordinate activities and routines with people based on their hopes and dreams.
  • Share new and/or updated information regarding people receiving services with teammates (medical, emotional, behavioral, etc.).
  • Perform medication administration, medication counts, etc.
  • Perform household tasks (cleaning, grocery shopping, etc.), as needed per individual.
  • Assist in ongoing training of employees and the training of new employees as directed by the Program Manager.
  • Collaborate with the Program Manager during staff training and department meetings.
  • Lead staff meetings as necessary.
  • Assist in the data collection of personal outcomes for FNRC quarterly reports.
  • Complete all financial month-end documentation (cash balance sheet, Credit Card reconciliation) by the required deadline.
