As a Supported Living Manager at Pathfinder, you play a critical leadership role in ensuring individuals receive high-quality, person-centered supports in their homes and communities. Serving as the bridge between residential sites and leadership, you guide teams, uphold compliance, and ensure each individual’s goals, preferences, and well-being remain at the center of everything you do. Requirements

  • Must have a valid driver's license.
  • Must have reliable transportation.
  • Must be able to pass a background check.
  • Must be able to pass a drug screening.

Responsibilities

  • Directly supervise and support Supported Living Assistant Managers through coaching, training, performance feedback, and corrective action when needed.
  • Lead recruitment, onboarding, and development efforts to build strong, reliable residential teams.
  • Serve as a role model by demonstrating ethical, stable, and compassionate leadership.
  • Participate in leadership on-call rotation to ensure continuity of care and operational support.
  • Oversee the implementation of Person-Centered Individual Support Plans (PCISPs) across assigned residential sites.
  • Ensure goals are measurable, meaningful, and aligned with individual preferences and outcomes.
  • Monitor and guide adherence to behavior support plans and provide direction on best practices.
  • Champion services that support physical, emotional, and social well-being.
  • Ensure all residential operations meet state and federal regulations, Medicaid Waiver requirements, and Pathfinder policies.
  • Oversee accurate documentation, waiver files, and timely reporting to leadership.
  • Maintain confidentiality, ethical standards, and agency compliance at all times.
  • Oversee day-to-day operations across three residential sites supporting 9–12 individuals.
  • Partner with Supported Living Assistant Managers to ensure staffing coverage, scheduling adjustments, and open-shift coverage.
  • Coordinate medical appointments, transportation, and day program schedules to support continuity of care.
  • Manage resources and staffing needs to maintain high operational standards.
  • Serve as the primary point of contact for interdisciplinary team members.
  • Foster strong relationships with families, guardians, and community partners.
  • Promote inclusion, communication, and quality of life for individuals supported.
