Requirements

  • Bachelor's degree in Human Services, Psychology, Special Education, or a related field preferred.
  • 3–5 years of experience in human services, preferably supporting individuals with intellectual and developmental disabilities.
  • Knowledge of person-centered planning, support coordination, and Rhode Island service systems preferred.
  • Strong communication, organization, advocacy, and relationship-building skills.
  • Proficiency with Microsoft Office, Google Workspace, and electronic documentation systems.
  • A genuine commitment to person-centered services and the belief that every individual has unique strengths, valuable contributions to make, and the ability to achieve meaningful life outcomes.

Responsibilities

  • Build strong relationships with individuals and families while developing person-centered Individual Support Plans (ISP/PCPs) that reflect each person's goals, strengths, and preferences.
  • Coordinate services and community resources that promote independence, inclusion, and an enhanced quality of life.
  • Collaborate with individuals, families, Conflict-Free Case Management (CFCM), providers, and interdisciplinary teams to ensure supports are coordinated, responsive, and effective.
  • Monitor services, advocate for individuals' rights, and help navigate changing needs and opportunities.
  • Educate individuals and families about available resources and supports while ensuring services remain person-centered.
  • Maintain accurate documentation and compliance with agency and Rhode Island regulations.
  • Participate in ongoing professional development and contribute to a collaborative, supportive team culture.
