The Department of Mental Health, Substance Abuse and Developmental Services seeks a Support Coordinator with the Developmental Services Division. This position is responsible for assessing, coordinating, monitoring, and linking service delivery for children and/or adults with intellectual and developmental disabilities and their families. Experience with the HCBS waivers, CCC Plus Waiver, and Medicaid is preferred. Additionally, knowledge of community-based resources including integrated housing options and employment first principles is desired. The Support Coordinator is responsible for providing high-quality customer service consistent with the mission, vision, and values of MHSADS.

Requirements

• Bachelor's degree in Human Services, Child Development, Early Childhood Special Education, Social Work, Counseling, Psychology, or related field and three (3) years of related experience in assigned program area or equivalent combination of education and experience.
• Demonstrate effective oral and written communication skills.
• Have strong organizational and time management skills.
• Have proficiency in utilizing electronic health records and other software programs for documenting service delivery.
• Valid driver's license in Virginia or regional state of residence.

Nice To Haves

• Master's degree in Human Services, Child Development, Early Childhood Special Education, Social Work, Counseling, Psychology, or related field.
• Three (3) years of related experience in assigned program area as a case manager, service coordinator, or multi-disciplinary team facilitator with children, youth, adults, and families.

Responsibilities

• Assessing eligibility and the needs of individuals.
• Assessing possible individual risks.
• Interviewing individuals and their families.
• Facilitating team meetings.
• Coordinating service delivery among providers and developing and documenting person-centered plans using a strengths-based approach.
• Assuring quality service provisions.
• Maintaining compliance with all State of Virginia Licensure standards, Medicaid standards, and crisis intervention and prevention.
