About The Position

This position is located at the John D. Dingell VA Medical Center (JDDVAMC). The incumbent is a Supervisory Social Work Program Coordinator and serves in a full-time capacity within the Mental Health Service (MHS) within the VA Detroit Healthcare System. The social worker is part of the Mental Health Homeless Programs Section (MHHP). The incumbent is responsible for the oversight of the Housing and Urban Development - Veteran Affairs Supportive Housing (HUD VASH) program at the facility.

  • Licensure/Certification. Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level.
  • Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE).
  • One year of experience equivalent to the GS-12 grade level.
  • Experience must demonstrate possession of advanced practice skills in administration, demonstrating progressively more professional competency skills and judgment.
  • Individual may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience.
  • Individuals assigned as social worker program coordinator must be able to provide supervision for licensure.
  • Skill in coordinating and implementing one or more specialty program(s), across the facility departments, and quality improvement.
  • Skill in local policy and practice development, procedures, and/or practice guidelines for the program as prescribed by the program handbook or national directive.
  • Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s).
  • Ability to collaborate with internal and external partners to further program goals and enhance patient centered care.
  • Ability to provide the full range of supervisory duties including assignment of work to be performed; performance evaluations; and selection of staff, training, and recommendation of awards, advancements, and when appropriate, disciplinary action.
  • Ability to oversee the fiscal matters of the functions supervised (including fund controls, contracts, and equipment expenditures), forecast resource and equipment needs, and administer the allocated budget.

Responsibilities

  • Develops, implements, and evaluates clinical guidelines and protocols for the HUD VASH Program.
  • Facilitates community organization and outreach to alleviate problems that may interfere with Veterans' ability to achieve and/or maintain permanent housing.
  • Supervises HUD VASH supervisors and HUD VASH team leads and has programmatic management responsibility for affiliated disciplines: occupational therapy, vocational rehabilitation, and peer support.
  • Provides executive oversight of the HUD VASH program as defined in prevailing Homeless Program Office (HPO) and Veteran Affairs Central Office (VACO) directives and recommendations.
  • Represents the VA at community meetings, seeking or maintaining members on relevant boards, councils, and task forces.
  • Plans, establishes, reviews, implements, and maintains policies, procedures, and protocols that satisfy accrediting guidelines (e.g. CARF, Joint Commission, VA-OIG, state regulation, etc.).
  • Facilitates the development of HUD VASH program goals and objectives.
  • Ensures the impartation and adherence to local directives and/or guidelines that pertain to HUD VASH specifically and mental health generally.
  • Participates in HUD VASH program conference calls, webinars, in-person seminars, and all training opportunities as appropriate.
  • Establishes and maintains a working relationship with Public Housing Authorities (PHA).
  • Audits/reviews VA HUD VASH records and PHA records to ensure congruency and accuracy.
  • Conducts ongoing and intensive reviews and appraisals to ensure the accomplishment of goals and objectives.
  • Completes gaps analysis as assigned and/or required.
  • Creates, monitors, and updates Operation Plans both locally for the medical center and within the Homeless Programs database for VA Central Office.
  • Establishes a process for referrals and admissions to the HUD VASH program.
  • Collects and submits HUD VASH program participant data as outlined by VA Central Office and local Continuum of Care/Medical Center agreements.
  • Completes all HUD VASH suspense actions and reports.
  • Serves as an advocate on behalf of Veterans to ensure comprehensive service delivery.
  • Complies with criminal conflict of interest laws and Executive Branch Standards of Conduct, avoiding conflicts of interest.
  • Identifies shifting trends with caseloads and updates program policies and procedures based on work experience.
  • Works with Homeless Programs Section Chief, MH Service Chief, and VA Detroit Executive Leadership Team to develop more effective and efficient ways of delivering healthcare services.
  • Coordinates interdisciplinary staffing and work assignments.
  • Provides clinical and case management services to Veterans.
  • Conducts biopsychosocial assessments and assessment updates.
  • Assesses the need for referral for more intensive and/or other mental health services.
  • Consults, collaborates, and coordinates with other staff who provide care to Veteran.
  • Participates in treatment team meetings.
  • Documents all clinical contact as warranted including notations in the electronic health record.
  • Provides clinical supervision to staff, health trainees, residents, and interns.
  • Must have a Valid Driver's license.
