About The Position

Supervises professional social work staff; performs psychosocial assessment of patients and families. Formulates and implements psychosocial treatment plans; identifies unmet psychosocial needs and develops programs to meet those needs. Participates in educational activities, professional organizations and hospital committees.

Requirements

  • Master of Social Work required.
  • Current License to practice as a Clinical Social Worker in New York State required, plus specialized certifications as needed.
  • 4-6 years of relevant experience required.
  • 0-2 years of leadership / management experience required.

Responsibilities

  • Promotes Social Work Services department goals by selecting, motivating, and training capable team members.
  • Leads the activities of assigned Social Work Services team members by communicating and providing guidance toward achieving department objectives.
  • Provides management and leadership to team members of Social Work Services by communicating and guiding toward achieving department objectives.
  • Develops, communicates, and builds consensus for goals in alignment with system.
  • Promotes department goals by selecting, motivating, and training capable team members.
  • Leads the activities of assigned team members by communicating and providing guidance toward achieving department objectives.
  • Assists in screening applicants for social work positions.
  • Assists staff in formulating and implementing psychosocial treatment plans based on patient and family needs.
  • Meets with staff social workers to review caseloads; assists staff in examining clinical, administrative and ethical issues.
  • Provides clinical supervision as applicable.
  • Monitors time, charting and statistics of social work staff in accordance with departmental criteria; participates in interdisciplinary treatment planning conferences.
  • Performs psychosocial assessments of patients and families; identifies patients needing psychosocial services.
  • Performs competency assessment/performance appraisals according to institutional schedules and policies.
  • Interviews patients and families, when necessary, to assess family structure, dynamics, adjustments to illness and access to necessary resources; identifies medical problems and predicts course; educates patients and families about community resources and discharge and planning procedures.
  • Participates in internal and external committees and workgroups that require social work expertise and best practice input.
  • Performs related duties as required.
