Supervisor - Integrated Case Management
About The Position
The Supervisor of Integrated Case Management oversees daily case management operations at Henry Ford Health's West Bloomfield Hospital and guides a team of professionals in delivering collaborative, patient-centered care. The role ensures the team effectively assesses, plans, coordinates, and evaluates care to meet the comprehensive health needs of individuals and families while promoting patient safety, quality of care, and cost-effective outcomes. The position is part of the Population Health team in a hospital setting. The supervisor contributes to policies, procedures, workflow design, and performance improvement activities, and works with an interdisciplinary care team to support optimal patient outcomes.
Requirements
- Bachelor's degree in nursing or a related professional field (such as social work, counseling, or health education), or a Master of Social Work degree.
- Three years of clinical experience in case management, disease management, health coaching, nursing, or home health care.
- Strong communication and interpersonal skills.
- Strong analytical, problem-solving, and critical thinking skills.
- Registered Nurse (RN) or Licensed Master Social Worker (LMSW) with a valid, unrestricted State of Michigan license.
Nice To Haves
- Certification in Case Management (CCM) from the Commission for Case Management Certification (CCMC).
Responsibilities

- Monitor and guide case management professionals in assessment, planning, facilitation, care coordination, evaluation, and advocacy.
- Participate in developing policies, procedures, and daily workflow design.
