About The Position

NYU Langone Hospital—Long Island is seeking a Supervisor, Home Care Social Work (LCSW) for its Garden City Location. This role involves providing professional supervision, clinical guidance, and assistance to the Social Workers of the Home Health Agency. The successful candidate will collaborate with leadership on program development, interdepartmental projects, and initiatives. This position also requires acting as a patient advocate, working with community partners and resources to enhance services, and developing best practice protocols and coordinating resources for all age groups and populations.

Requirements

  • Masters Degree in Social Work (MSW), or a higher degree from a program accredited by the Council on Social Work Education (CSWE) and recognized by the New York State Education Department as qualifying for licensure, is required.
  • Licensure as a Licensed Clinical Social Worker (LCSW) in New York State is also required.
  • A minimum of five years of progressive clinical experience in social work is required.
  • At least one year of experience in a supervisory, administrative, or social work teaching role is also necessary.
  • Knowledge of psychosocial assessments, discharge planning, community resource linkage, and behavioral health interventions is essential.
  • Excellent verbal and written communication, interpersonal, leadership, and problem-solving skills.
  • Strong organizational and team-building abilities are critical.
  • Proficiency in clinical documentation and electronic health record systems.
  • Ability to manage and support a multidisciplinary team in delivering patient-centered care.
  • Qualified candidates must be able to effectively communicate with all levels of the organization.

Nice To Haves

  • Preference for those who have worked in healthcare, hospital, or home health settings.
  • In certain cases, a limited permit issued by the New York State Education Department may be considered, depending on departmental needs.
  • Familiarity with regulatory requirements and standards in acute care, home health, or oncology settings is expected.
  • 1 - 2 years Management in Home Health.

Responsibilities

  • Performs other duties as assigned
  • Provide oversight for clinical and professional practice for all home care social workers.
  • Provide direct oversight for clinical competencies.
  • Review clinical documentation for compliance with standards and regulations.
  • Participate in hospital committee meetings, community events, networking and outreach.
  • Develop and implement specialty programs and best practice protocols for disease management, wellness and population management.
  • Serve as a role model and liaison as part of interdisciplinary team in the agency and organization.
  • Facilitate communication with team members, community resources, professional organizations and affiliations.
