This is a grant-funded position. The Talent Acquisition Partner will be able to provide more details in the form of a Grant Funded Position Agreement. The position's purpose is to provide supervision, coaching, and direct support to a team of Community Health Workers (CHWs) working to improve health outcomes and eliminate racial, ethnic, and economic disparities in those outcomes. The role plays an integral part in program management, fostering the growth of CHWs, ensuring quality of service delivery, and guiding progress towards intervention goals. The Supervisor is responsible for the daily functions of a team of CHWs, including health system navigation, health education, patient engagement and follow-up, resource advocacy, and outreach activities. They will also assist with program design, evaluation, and evolution.
