About The Position

HealthRIGHT 360 is a nonprofit organization dedicated to providing accessible and comprehensive healthcare services to vulnerable populations. Our mission is to address systemic barriers to healthcare and promote health equity. We offer a range of services including mental health care, substance use treatment (residential and outpatient), and primary health services, as well as transitional support for individuals re-entering the community. We integrate physical and behavioral health to help individuals overcome challenges by addressing social determinants of health, fostering resilience, and facilitating recovery. Prototypes Women's Center in Pomona is a 180-bed residential facility specifically for women, including those who wish to involve their children in their recovery. The program supports women in overcoming co-occurring disorders through various services. Children can develop secure attachments in a safe environment while accessing mental health support and educational assistance. Our interdisciplinary team provides comprehensive treatment for substance use disorders, mental health, parenting, education, and life skills. Treatment duration can range from 60 days to 6 months.

Requirements

  • High school diploma or equivalent required.
  • Registration as a Substance Use Disorder Counselor from an accredited California agency (CCAPP, CAADE, CADTP).
  • Valid First Aid and CPR certification or ability to obtain within 30 days of hire.

Nice To Haves

  • Bachelor’s degree in Health and Human Services, Social Work, Psychology, or other related field.
  • Experience as a registered substance abuse or behavioral disorder counselor or recovery specialist in community mental health or residential setting.

Responsibilities

  • Provide individual counseling sessions to clients on an as-needed basis, in alignment with existing treatment plans and program requirements.
  • Facilitate group counseling sessions and community meetings as scheduled or in place of absent staff.
  • Support client engagement by linking them to internal and external resources based on identified treatment needs.
  • Deliver educational classes and structured programming on treatment-related topics to support client recovery goals.
  • Collaborate with treatment team members to review, update, and maintain treatment and transition plans, progress notes, and other clinical documentation, as appropriate.
  • Perform crisis intervention and communicate critical updates to the treatment team to support client stability.
  • Monitor and document client progress during coverage periods and ensure thorough handoff to regular staff.
  • Assist in the monitoring and documentation of client medication administration when applicable.
  • Ensure secure handling and proper documentation of medications while on duty.
  • Accompany clients to offsite appointments when required and authorized.
  • Assign client duties and sign off upon completion, ensuring clients stay engaged in their treatment process.
  • Motivate clients to fulfill their weekly obligations and stay on track with their treatment goals.
  • Support the intake process, ensuring all required documentation and supplies are maintained for smooth operation.
  • Collect, clean, and dispose of biohazard waste in accordance with program policies.
  • Disinfect and clean client rooms, common areas, and program spaces to ensure a safe, sanitary environment.
  • Maintain safety and security of clients in the facility.
  • Perform periodic house runs.
  • Perform property and room searches.
  • Respond to medical emergencies including overdoses.
  • Perform substance use tests such as urinalysis and breathalyzer.
  • Observe clients and maintain observation logs.
  • Document group and individual counseling sessions in a timely manner per program and/or contract requirements.
  • Accurately enter data in electronic systems.
  • Document client updates and incidents daily.
  • Complete and maintain all documentation in accordance with guidelines established by HealthRIGHT 360 to satisfy internal and external evaluating requirements.
  • Complete discharge paperwork and agency assessments.
  • Maintain logs of client contacts.
  • Read and respond to communications in a timely manner.
  • Attend and actively participate in meetings.
  • Meet residential service delivery goals.
  • Plan and facilitate celebrations and special events.
  • Work on weekends and holidays as needed.
  • Complete assigned training in a timely manner.
  • Maintain full compliance with credential requirements.
  • Perform other assigned duties.
