The Substance Abuse Counselor provides direct care services to patients with mental, emotional, and substance abuse issues, working closely with the treatment team to fulfill the interdisciplinary treatment process for patients with a dual diagnosis. This includes developing and conducting program activities for education and treatment related to substance abuse, as well as providing individual and group counseling sessions. The counselor is responsible for patient assessment and the development and implementation of Recovery Plans, within the company's standards. You’ll conduct assessments and intakes, develop treatment plans, lead group counseling sessions, and provide individual support to patients at varying stages of recovery. Much of the work centers on group facilitation, behavior change, and relapse prevention, while also addressing co-occurring mental health needs. You’ll document care in the medical record, track progress, and ensure compliance with standards. A key part of the role is preparing patients for reentry by connecting them to community resources and building realistic plans for continued treatment after release, making a direct impact on long-term outcomes.
