About The Position

As the Stroke Coordinator, you will provide expert clinical leadership, coordination and consultancy to strengthen stroke care across the Limestone Coast Local Health Network. Working across disciplines, services and care settings, you will support evidence-based stroke management, coordinate complex care pathways and drive improvements in patient outcomes. You will work closely with clinicians, leaders and key stakeholders to strengthen clinical practice, education and service development, while monitoring outcomes and supporting compliance with contemporary stroke standards. This is an opportunity to influence stroke care at a regional level and contribute to LCLHN’s progress towards Stroke Capable Regional General Hospital status and achievement of the 30/60/90 National Stroke Targets.

Requirements

Hold the essential professional qualifications and registration applicable to your classification – RN/M3 applicants must be registered or eligible for registration as a Nurse with the Nursing and Midwifery Board of Australia and hold, or be eligible to hold, a current practising certificate; AHP4 applicants must hold a recognised qualification in a relevant Allied Health profession and meet applicable professional association and/or AHPRA registration requirements.
For RN/M3 applicants, have at least three years' post-registration experience and demonstrated competence in the relevant area of nursing practice.
Demonstrate advanced clinical leadership and professional judgement, with the ability to provide expert advice, consultation and support in complex clinical environments.
Bring highly developed communication, collaboration and negotiation skills, with the ability to build effective relationships across multidisciplinary teams, health services and external organisations.
Demonstrate experience in quality improvement, service development, education and evidence-based practice, with the ability to influence positive change and improve clinical outcomes.
A current driver's licence is essential, with a willingness to drive on country roads and travel as required.

Nice To Haves

Postgraduate qualifications in neurology/stroke at Graduate Diploma or Master's level are desirable for RN/M3 applicants.
