About The Position

Southwest General Home Health is seeking a Home Health Aide to provide compassionate, supportive care to patients in their homes. Working under a nurse’s supervision, the aide helps patients feel safe and comfortable while following each patient’s plan of care and working within their scope of practice. The schedule is Monday through Friday, 8:30 AM to 5:00 PM. The role involves traveling to scheduled patient visits and completing visit documentation within 24 hours. Duties and responsibilities may change or be assigned at any time.

Requirements

• High school diploma or equivalent.
• State Tested Nurse Aide (STNA) credential or completion of a 75-hour nurse aide training program (CNA).
• One year of verifiable nurse aide experience under an RN’s direct supervision.
• Score of 80% or higher on the skills competency test.
• Valid driver’s license and auto insurance listing the applicant as a driver.
• Ability to safely perform hands-on care, including lifting, transferring, and bathing.
• Caring attitude, strong communication skills, and dependable attendance.

Responsibilities

• Assist patients with bathing, grooming, toileting, and other personal care needs.
• Help with meals, laundry, and light housekeeping as needed.
• Support safe transfers, walking, and prescribed exercises.
• Observe and report changes in patients’ conditions.
• Complete visit documentation within 24 hours.
• Travel to scheduled patient visits and follow each patient’s plan of care within the aide’s scope of practice.
• Complete required training, including 12 hours of annual in-services.
