About The Position
Prescott Therapies and Prescott Home Health are seeking a start of care nurse to cover the twin cities metro area. They offer hourly pay, valuing the time and effort put into thorough assessments, complex documentation, and delivering exceptional patient advocacy. Prescott Therapies is a locally owned, mission-driven practice serving adults with developmental disabilities across the Twin Cities metro. Their interdisciplinary team provides Physical, Occupational, and Speech Therapy, and they are expanding their nursing leadership to better serve clients in group homes and home health settings. The role offers a collaborative culture and work-life balance with flexible scheduling and no weekends.
Requirements
- Current MN Registered Nurse (RN) License.
- A "big picture" mindset—you enjoy the puzzle of a complex admission.
- Strong rapport-building skills with a diverse caseload.
- Excellent clinical documentation skills and attention to detail.

- OASIS documentation experience beneficial
Responsibilities
- Conduct comprehensive Start of Care (SOC) assessments and admissions.
- Develop individualized, holistic plans of care in collaboration with the clinical team.
- Perform routine nursing assessments and medication reconciliations.
- Coordinate with physicians, families, and group home staff to ensure seamless care transitions.
