Requirements

  • Bachelor of Science in Pharmacy required
  • Current licensure as a Registered Pharmacist in the State of New York.
  • Experience and strong knowledge base in intravenous admixture services and intravenous therapy.
  • Ability to evaluate and monitor drug therapy relative to the total needs and clinical conditions of home care patients in collaboration with physicians, nurses, and other healthcare professionals.
  • Ability to work effectively with interdisciplinary teams to provide safe and effective drug therapy.
  • Demonstrated ability to problem solve and supervise pharmacy technicians and support staff.
  • Strong organizational and time management skills.
  • Excellent verbal and written communication skills.
  • Demonstrated dependable attendance and consistent job performance.

Nice To Haves

  • Doctor of Pharmacy (PharmD) or Master of Science in Pharmacy preferred.
  • Minimum of two years of recent experience in an acute care setting or one year of recent home infusion therapy experience preferred.
  • Participation and membership in professional pharmacy societies and organizations preferred.

Responsibilities

  • Review infusion orders and patient therapies
  • Participate in clinical rounds
  • Perform sterile compounding and TPN review
  • Maintain patient records and care plans
  • Ensure USP and cleanroom compliance
