Requirements
- Bachelor of Science in Pharmacy required
- Current licensure as a Registered Pharmacist in the State of New York.
- Experience and strong knowledge base in intravenous admixture services and intravenous therapy.
- Ability to evaluate and monitor drug therapy relative to the total needs and clinical conditions of home care patients in collaboration with physicians, nurses, and other healthcare professionals.
- Ability to work effectively with interdisciplinary teams to provide safe and effective drug therapy.
- Demonstrated ability to problem solve and supervise pharmacy technicians and support staff.
- Strong organizational and time management skills.
- Excellent verbal and written communication skills.
- Demonstrated dependable attendance and consistent job performance.

Nice To Haves
- Doctor of Pharmacy (PharmD) or Master of Science in Pharmacy preferred.
- Minimum of two years of recent experience in an acute care setting or one year of recent home infusion therapy experience preferred.
- Participation and membership in professional pharmacy societies and organizations preferred.
Responsibilities
- Review infusion orders and patient therapies
- Participate in clinical rounds
- Perform sterile compounding and TPN review
- Maintain patient records and care plans
- Ensure USP and cleanroom compliance
- Train and support staff
- Drive operational improvements
