We fulfill this mission through: • Improving access to primary health care for residents of South Bay • Employing skilled professionals committed to providing quality healthcare services and addressing current health needs • Serving the community with compassion and respect while maintaining confidentiality • Collaborating with other health care providers and organizations • Providing a medical home to the "at-risk" population Staff Clinical Registered Nurses (RN) work within a patient-centered team-based care environment to provide excellent patient care services within scope of practice as established by the Nurse Board of California. RN are members of patient-centered care teams. RN create access to care for patients by determining acuity of patient care when access is limited by providing excellent communication with patients. RN develop and maintain relationship, communication, and nursing skills with the overall goal of providing excellent patient-centered treatment, care, and services in a team environment. Services including routine primary care at a FQHC may include but not limited to adult, pediatric, gynecology, prenatal, and behavioral health. This role reports to the Nurse Manager.

Requirements

  • Graduate of an accredited nursing program; Bachelor of Science in Nursing preferred.
  • Current, active California Registered Nurse license.
  • Current BLS/CPR certification.
  • Minimum two years of nursing experience; community health or FQHC experience preferred.
  • Bilingual English/Spanish
  • Strong assessment, triage, organizational, and communication skills.
  • Proficiency in electronic health record documentation.
  • Ability to manage competing priorities, work collaboratively, and respond calmly to urgent situations.
  • Compliance with IBCC employment health requirements, policies, and professional standards.

Nice To Haves

  • Bachelor's degree from four-year college or university preferred
  • Experience in FQHC preferred.
  • Bilingual English/Spanish

Responsibilities

  • Assess and triage patients in person and by telephone, determine urgency, and coordinate timely care with providers.
  • Provide nursing visits, treatments, medications, and immunizations under authorized provider orders or approved protocols.
  • Perform wound care, catheter and tube care, specimen collection, and suture or staple removal within scope and competency.
  • Review laboratory results, promptly communicate abnormal or critical findings to providers, and coordinate follow-up.
  • Support medication refills, pharmacy communication, and medication clarification according to clinic protocols.
  • Coordinate referrals, emergency department and hospital follow-up, and services for patients with complex medical or social needs.
  • Educate patients and families on medications, chronic conditions, preventive care, immunizations, and treatment plans.
  • Respond to medical emergencies, provide BLS/CPR, and assist with emergency transfers.
  • Document assessments, care, patient responses, and follow-up accurately and promptly in the electronic health record.
  • Collaborate with providers, medical assistants, and other departments; provide nursing guidance and support safe clinical workflows.
  • Participate in team huddles, staff training, quality improvement, and patient-centered care initiatives.
  • Maintain patient confidentiality, infection prevention practices, safety standards, and required clinical competencies.
  • Perform other duties as assigned within scope of practice and competency.
  • Assist with report submission as dictated
  • Reports any clinical concerns or safety issues to clinic managers or directors
  • Performs other related duties as assigned or requested by Nurse Manager or leadership team members
  • Maintains facility management by abiding by quality assurance, safety, environmental, infection control standards including following infection disease precautions to prevent the spread of organisms.
