About The Position

Making Opportunity Count (MOC) is dedicated to empowering families to achieve economic security by eliminating barriers and creating opportunities. MOC addresses poverty by providing a range of services, including childcare, education, workforce development, nutrition, health, and community and housing support. The Stabilization Case Manager will provide ongoing case management services to families after they have moved from shelter into their own apartments. The primary focus of this role is continuous barrier reduction to ensure the family's long-term success and to help them develop the skills and resources needed to sustain housing. Families typically participate in the Stabilization Program for up to three years.

Requirements

Minimum two (2) years of experience
Associate’s or Bachelor’s degree in Human Services, or related field
Minimum 7 years of human service experience.

Responsibilities

Monitor intake, assessment, and data entry in a timely manner for all program clients, utilizing appropriate forms and assessment tools.
Maintain regular communication with the Property Owner to ensure program compliance.
Assist families with tenancy concerns, such as developing a budget plan, reporting maintenance concerns to the property owner, and addressing reported behavior concerns.
Assist families with enrolling in entitlement benefits (SNAP, TAFDC, SSI).
Work collaboratively with community partners.
Conduct regularly scheduled meetings with families.
Keep supervisor aware of any concerns or potential problems.
Attend appropriate meetings and trainings as needed.
Perform all other duties as assigned by the Shelter Manager and/or the Director of Shelter Case Management.
