About The Position
The Outreach Coordinator conducts outreach to Veterans and community members experiencing or at risk of homelessness, linking them to rapid rehousing, homelessness prevention, and supportive services.
Requirements
- High School Diploma required.
- Valid driver license, current vehicle insurance and registration, satisfactory motor vehicle report, and successful criminal history check.
Nice To Haves
- Community Health Worker certification preferred.
- Experience in outreach, housing, homeless services, or veteran services preferred.
- Veterans preferred.
Responsibilities
- Conduct outreach services and street outreach activities.
- Connect participants with case management, benefits, and housing resources.
- Verify homelessness status and eligibility.
- Develop relationships with landlords, shelters, VA offices, GPD providers, CoCs, and community partners.
- Assist with transportation, communication, self-advocacy, and housing support plans.
- Attend Stand Down and community outreach events.
- Collaborate with SSVF staff and perform other assigned duties.
