Join our team as the Spiritual Care Coordinator. In this role you will be responsible for providing spiritual care to clients and families. You will also participate as a member of the Hospice interdisciplinary group. We are seeking a compassionate and collaborative professional to support hospice patients and families by coordinating referrals and home or facility visits while working closely with an interdisciplinary care team. This role requires strong communication skills, timely documentation, and a deep understanding of hospice philosophy, with a focus on delivering high-quality, patient-centered care. The ideal candidate is comfortable addressing grief and loss, providing counseling and bereavement follow-up services, and supporting both staff and families through emotionally complex situations. Responsibilities also include engaging with clergy and community groups to address spiritual needs, facilitating memorial services when appropriate, and contributing to staff education and patient care conferences. Additionally, the position plays an active role in quality improvement initiatives and community bereavement programs, ensuring compliance with all regulatory and organizational standards.

Requirements

  • Graduate of an accredited seminary or school of theology or appropriate certification in hospital or pastoral ministry required.
  • Minimum one (1) year experience as an ordained pastor/rabbi or one (1) year of hospice experience
  • Valid state driver’s license.
  • Ability to successfully pass pre-employment driver record, criminal, adult, and child abuse background, and Office of Inspector General (OIG) check/review.
  • Must show proof of auto insurance at time of employment.
