About The Position

Concordia Visiting Nurses is a Medicare-certified, state licensed, Medicaid approved home health agency operating in nine Western Pennsylvania counties. For well over a decade, Concordia Visiting Nurses has provided high-quality skilled nursing, palliative care, and a variety of therapy services to our patients. We're currently seeking a personable and motivated Speech Therapist ST to join our growing team. You’ll work 1:1 with adult and geriatric patients in the comfort of their homes—helping them reclaim communication, swallowing function, and confidence in their daily lives.

Requirements

• Current license as a Speech-Language Pathologist in (PA)
• ASHA CCC-SLP Certification
• 1+ years of experience (home health a plus but not required—we’ll train the right candidate!)
• Strong interpersonal and communication skills
• Reliable transportation and valid driver’s license

Responsibilities

• Provide skilled speech therapy services to patients in their homes
• Develop and implement personalized care plans
• Evaluate speech, language, cognition, voice, and swallowing disorders
• Educate patients and caregivers on therapeutic strategies
• Collaborate with Nurses, PT/OTs, and physicians to ensure the best outcomes
