About The Position

Concordia Visiting Nurses is seeking a personable and motivated Speech Therapist (ST) to join their growing team. This clinician-led home health agency values heart, integrity, and personalized care. The Speech Therapist will work 1:1 with adult and geriatric patients in their homes, helping them to reclaim communication, swallowing function, and confidence in their daily lives. The role offers a flexible schedule, autonomy with support, the opportunity to build meaningful relationships with patients and families, and local routes within a preferred travel area. The team is described as caring and supportive.

Requirements

• Current license as a Speech-Language Pathologist in (PA)
• ASHA CCC-SLP Certification
• 1+ years of experience (home health a plus but not required—we’ll train the right candidate!)
• Strong interpersonal and communication skills
• Reliable transportation and valid driver’s license

Responsibilities

• Provide skilled speech therapy services to patients in their homes
• Develop and implement personalized care plans
• Evaluate speech, language, cognition, voice, and swallowing disorders
• Educate patients and caregivers on therapeutic strategies
• Collaborate with Nurses, PT/OTs, and physicians to ensure the best outcomes
