The speech-language pathologist is responsible for implementation of standards of care for speech-language pathology services and for adherence to all conditions in the Service Agreement. The position involves conducting evaluations, assessing home environments, identifying equipment needs, and establishing plans of care to improve patient function. The role includes administering speech therapy programs, recommending communication devices, and collaborating with physicians and other personnel for coordinated care. The speech-language pathologist also participates in case conferences and other meetings, prepares clinical notes, and maintains competency in the field. Additionally, the role involves identifying patient and family needs for other services, referring as appropriate, and instructing patients, families, and other healthcare personnel in treatment regimens. The speech-language pathologist may supervise home health aides in procedures directly related to the speech-language therapy plan of care and performs other duties as required.
