About The Position

Human Touch Home Health is seeking a compassionate Speech-Language Pathologist (SLP) for PRN home health opportunities throughout Washington, DC. This role offers flexible scheduling, one-on-one patient care, and the opportunity to make a meaningful difference in communication, cognition, and swallowing. For more than 27 years, Human Touch Home Health has provided skilled and non-skilled home health services throughout Washington, DC, Maryland, Virginia, and Pennsylvania. Requirements

Active Washington, DC Speech-Language Pathologist License Required
Master's Degree in Speech-Language Pathology from an accredited program
Strong Clinical Assessment and Communication Skills
Dependable Transportation to Patient Visits
Ability to Work Independently in Patients' Homes

Nice To Haves

Home Health Experience Preferred, but Not Required
WellSky Experience is a Plus

Responsibilities

Complete speech therapy evaluations in patients' homes
Develop individualized treatment plans
Evaluate and treat speech, language, cognitive, communication, and swallowing disorders
Provide interventions to improve functional communication and independence
Educate patients and caregivers on treatment strategies and safety
Monitor patient progress and adjust treatment plans as needed
Coordinate care with physicians, nurses, therapists, and other team members
Complete progress assessments and discharge planning
Complete accurate and timely documentation using WellSky
