About The Position

Human Touch Home Health is seeking a compassionate Speech-Language Pathologist (SLP) for PRN home health opportunities throughout Fredericksburg and surrounding communities. Patient opportunities may include Fredericksburg, Stafford, Spotsylvania, King George, Caroline County, Orange, Locust Grove, Bowling Green, Colonial Beach, and nearby areas. You do not have to cover the entire territory. Choose the areas that work best for you and build your caseload around your availability. Speech therapy at home is about more than words. You're helping someone communicate their needs, safely enjoy a meal, strengthen cognitive skills, and regain confidence in everyday life. Home health allows you to see the real-world impact of your treatment while providing focused, one-on-one care.

Requirements

Active Virginia Speech-Language Pathologist License Required
Master's Degree in Speech-Language Pathology or related field
Valid Driver's License and reliable transportation
Strong clinical assessment and communication skills
Ability to work independently in patients' homes

Home Health Experience Preferred, but Not Required
WellSky Experience is a Plus

Responsibilities

Complete speech therapy evaluations in patients' homes
Develop individualized plans of care
Evaluate and treat speech, language, cognitive, communication, and swallowing disorders
Provide dysphagia treatment and swallowing education as appropriate
Help patients improve communication and functional independence
Educate patients, families, and caregivers
Monitor progress and adjust treatment plans as needed
Coordinate care with physicians, nurses, therapists, and other team members
Complete accurate and timely documentation using WellSky
Participate in reassessments and discharge planning
