The Speech–Language Pathologist is responsible for the implementation of standards of care for speech language pathology services. This role involves conducting evaluations, assessing home environments, identifying equipment needs, and developing individualized plans of care in partnership with patients and caregivers. The position requires providing ordered services, patient and caregiver education, counseling, and preparing clinical notes. Communication with physicians and other healthcare practitioners regarding the home health plan of care is essential. The therapist will identify functional speech defects, establish plans of care, administer therapy programs using specialized techniques and equipment, and recommend communication devices. Participation in case conferences, in-services, and other meetings is expected to ensure coordinated care. Maintaining clinical competency, identifying patient and family needs for other services, and referring as appropriate are key responsibilities. The role may involve supervising home health aides in therapy-related procedures and participating in the agency's quality assessment and performance improvement program. The position requires performing other duties as needed to facilitate speech language pathology services.
