About The Position

We are seeking a true PRN team member who can provide coverage during vacations, sick calls, and other staffing needs as they arise. Join an amazing in-house team dedicated to enriching the lives of seniors through compassionate care and innovative programs. As a Speech Language Therapist, you’ll provide exceptional services within Assisted Living, Skilled Nursing, and Memory Care residents. This role will foster a positive, supportive environment for residents, families, and team members on a PRN basis. This role primarily works in inpatient Med A.

Requirements

A minimum of a Master’s Degree in Speech Language Pathology.
CPR certified.
Licensed by the State in the designated geographic area in their respective therapy profession.

Responsibilities

Responsible for the maintenance and accuracy of records regarding patient care for all patients on personal case load.
Responsible for the correct reporting of billing information of Speech Therapy regarding all patients on personal case load.
Provide direct patient care as prescribed, including evaluation and treatment planning.
Responsible for the reporting and interpretation of treatment results.
Perform clinical functions: screens, evaluations, development of treatment plans, and delivery of treatment in accordance with facility, Medicare, and Therapy Management Corporation’s policies and procedures.
Participate in meetings regarding patient care as appropriate.
Maintains a high standard of resident care.
Provide education to patient’s family as well as staff on the specific treatment plan as well as therapy goals for the patient.
Responsible for the coordination and cooperation with other staff members in carrying out the treatment plan as established.
